Surgery is the most demanding and complex field of medicine. Decisions made in a matter of seconds can determine a patient's health and quality of life for years to come. Dr Tsvetelina Spiridonova, MD, knows this well. After graduating in medicine from the Medical University of Sofia in 2000, she spent three years specialising in surgery in Paris. In 2009, she became an Assistant Professor of Surgery at the Medical University of Sofia and, in 2015, was promoted to Chief Assistant Professor. Dr Spiridonova is an honorary member of the Bulgarian Surgical Society, a member of the German Society of Surgery, Chair of the Healthcare Committee at the Confederation of Employers and Industrialists in Bulgaria (KRIB) and Deputy Chair of the Bulgarian Hospital Association.


 

Yet surgery is only one aspect of the role medicine plays in Dr Spiridonova's life. She also holds a Master's degree in Healthcare Management from the Medical University of Sofia, knowledge she now applies as manager and owner of Higia Medical Facilities. Founded by her father, Prof. Stayko Spiridonov, Higia Hospital in Pazardzhik was one of Bulgaria's first private hospitals. Today, it has earned a reputation for putting patients first. The Higia brand has also expanded, with a medical centre now operating in Sofia.

When we last spoke, it was shortly after the end of the COVID-19 pandemic, which placed enormous pressure on the healthcare sector. What are the biggest challenges facing it today, and how is Higia responding?

Medicine has always had one fundamental challenge: to provide every patient who has placed their trust in us with high-quality care. Given the current level of healthcare funding, that can sometimes seem almost impossible. At Higia Hospital, we strive to maintain the highest professional standards while investing in timely technological innovation. The reality is that this often comes at the cost of compromises affecting our staff, whose greatest motivation remains their humanity and dedication. There is no real competition in medicine - only the constant pursuit of greater knowledge and experience. As a manager, my greatest challenge is to ensure that I am satisfied with the people who work in the hospital and that they, in turn, are happy with the way it is managed.

Higia recently celebrated its 35th anniversary. What have been the hospital's greatest achievements over those years?

Our greatest achievement is that 70% of our doctors and nurses have been working with us for more than 25 years. Staff turnover at Higia is exceptionally low. I can count on one hand the occasions when employees have left or when I have had to dismiss someone. There has always been a good reason, one that was incompatible with my understanding of medicine and the medical profession. At the same time, I am delighted that almost every month we welcome new doctors and nurses to the team. We continue to grow. At Higia, we have no staff shortages and our patients receive round-the-clock care.

And how do you see the hospital developing over the next five years?

I never talk about my business plans. I simply make them happen.

You have said that you are proud to run an honest, transparent and legitimate business. How is that possible in Bulgaria?

Bulgaria is a country of "impossible" scams. I have always been struck by the ingenuity behind the schemes people come up with and I have never understood how anyone could take satisfaction from them. When I meet people who claim they are doing business in that way, I quickly explain that business should be honest, transparent and legitimate. Everything else is crime. Anyone who looks into the history of Higia Hospital will see that every investment has been transparent and backed by documented sources of funding. Our financial flows are clearly defined and fully traceable. Neither my father nor I have ever allowed ourselves to turn our employees into accomplices by paying wages in cash envelopes under the table and drawing them into the grey economy. Running an honest and transparent business in Bulgaria, refusing to be corrupt and refusing to corrupt others, is ultimately a matter of dignity, despite all the risks that entails.

Patients at Higia know they will be welcomed by friendly, approachable staff. As a manager, how do you create such a high level of job satisfaction among your team?

I have never regarded my colleagues as employees. They are doctors, and I am a doctor. We are partners in running an excellent hospital. I would also like to emphasise that the nurses at Higia Hospital embody the professionalism and dignity that have long defined their profession. Working alongside our doctors, they understand that they are the true face of our teams in the eyes of our patients.

How do you decide who should join your team? What qualities do you look for in new colleagues?

I can usually recognise the right person within the first five minutes of our conversation. They are not tired of life or disillusioned with their profession. I look for people who are hardworking and committed, who bring new ideas and challenge me to give them opportunities to develop. Above all, I want them to be good people. Only then does their professional background come into play. Some nurses have worked in ten different hospitals, but they carry with them the frustration and poor habits they have picked up elsewhere. Even if I need staff, I will not hire them. On the other hand, some people arrive saying they are ready to do whatever it takes to work with patients and to learn the profession at its very best, with compassion, respect, discipline and teamwork. My door is always open to people like that.

You are both a doctor and a manager. Which of these roles gives you greater satisfaction? Is it difficult to combine them?

I do not know a single doctor who would answer that there is anything more rewarding than being a doctor. The only role that comes close is being a mother, although my son is becoming increasingly independent, so being a doctor remains the greatest privilege in my life. I have never denied that wearing the manager's "crown" can be a heavy burden. As a manager, sooner or later you have to make difficult disciplinary decisions. As a doctor, however, your duty is simply to do good.

Which is more difficult: making a decision in the operating theatre or making one that determines the future of an entire hospital?

Surgery is the pinnacle of medicine. I say that not because I am a surgeon, but because that is simply the reality. I am sure my colleagues in other specialties would agree. In the operating theatre, life-changing decisions often have to be made within 30 seconds. Those decisions carry responsibility for an entire lifetime. That is completely different from the responsibilities of management - ensuring financial stability, making investments, paying salaries and shaping business strategy. There are no decisions more profound than those made in the operating theatre. In many cases they are final, requiring not only knowledge and technical skill but also exceptional mental resilience.

How do you see the role of artificial intelligence in modern medicine? How is Higia preparing to introduce it into clinical practice?

Artificial intelligence is proving to be extremely valuable. I have been reading some fascinating research. There are AI-powered wearable devices that monitor patients' biological indicators and can predict acute events or relapses of chronic conditions, such as heart attacks, strokes, migraine attacks or episodes of Meniere's disease. AI is also transforming diagnostic imaging. We have an MRI scanner that uses AI to produce images three times faster and with around ten times greater accuracy than systems without AI. Artificial intelligence will become an integral part of medicine, providing the foundation for faster and more accurate clinical decisions. However, it will never replace the human element when it comes to caring for patients and making the final judgement.

Can a private hospital be both a successful business and put patients ahead of financial results?

Clearly it can. Higia has been doing exactly that for the past 35 years.

When you took over the management of Higia, you became the youngest owner and manager of a private healthcare institution in Bulgaria. How do you feel in your role today?

I am beginning to feel a little tired. Perhaps that is because being an owner means you are at work 24 hours a day, 365 days a year. I always smile when people ask me, "When do you go on holiday?" That question never fails to amuse me. And yes, for better or worse, challenge is simply my destiny.

Д-р Цветелина Спиридонова, д.м., мениджър и собственик, за предизвикателството да създаваш мотивиран екип и да даваш най-доброто на пациента, и за вечния чар на хирургията

Хирургията е най-предизвикателната и сложна част от медицината – решения, които се взимат за части от секундата, могат да предопределят здравето и живота на пациента за години напред. Д-р Цветелина Спиридонова, д.м., знае това добре. След като завършва медицина през 2000 г. в Медицински университет – София, тя специализира три години хирургия в Париж. През 2009 г. става асистент по хирургия към Медицински университет – София, а от 2015 г. – главен асистент. Д-р Спиридонова е почетен член на Българското хирургично дружество, член на Немското хирургично дружество, председател на комитета "Здравеопазване" към КРИБ и заместник-председател на Българската болнична асоциация.

Но хирургията далеч не изчерпва мястото, което медицината заема в живота на д-р Спиридонова. Тя завършва магистратура "Здравен мениджмънт" към Медицински университет – София и днес прилага на практика наученото като мениджър и собственик на лечебни заведения "Хигия". Завещана от нейния баща, проф. д-р Стайко Спиридонов, болница "Хигия" в Пазарджик е първата частна болница в Източна Европа. Днес тя има репутацията на място, в което грижата за пациента е най-важното. Но не само това – брандът "Хигия" се разраства и вече има голям многопрофилен медицински център и в София.

Предишният път, когато разговаряхме с вас, беше малко след края на пандемията от COVID-19, която изправи пред огромно предизвикателство здравния сектор. Кои са новите предизвикателства пред него днес и как "Хигия" се справя с тях?

Медицината винаги има само едно предизвикателство – изначално да осигуриш качествена здравна грижа на пациента, който ни се е доверил. В контекста на настоящото финансиране в здравеопазването, това е равностойно на чудеса. Болница "Хигия" се опитва да поддържа изключително високо професионално ниво с навременни технологични иновации. Истината е, че това не рядко е с цената на компромиси спрямо персонала, чиято мотивация основно остава хуманността. В медицината няма конкуренция, там говорим винаги за надграждане на знания, на опит. За мен, като мениджър, предизвикателство остава да бъда доволна от хората, които работят в болницата, и те да бъдат удовлетворени от начина на управление.

Наскоро "Хигия" отпразнува 35-тата си годишнина. Кои са най-големите постижения на болницата за това време?

Най-голямото ни постижение е, че 70% от лекарския и сестринския състав на болницата имат над 25-годишен трудов стаж именно при нас. В "Хигия" текучеството на кадри е изключително слабо. На пръсти се броят случаите, в които са напускали или съм уволнявала кадри. Винаги е било мотивирано и несъвместимо с моите представи за медицина и упражняване на лекарската професия. Но пък съм много щастлива, че почти ежемесечно имаме нови назначения и на лекари, и на сестри. Прирастът е позитивен. В "Хигия" няма недостиг на персонал и всички пациенти са денонощно обгрижени.

А как планирате да се развива тя през следващите пет години?

Никога не говоря за плановете си в бизнеса. Реализирам ги.

Споделяли сте, че се гордеете с един бял, чист и реален бизнес. Как се постига това в България?

България е страната на "невъзможните" мошеничества. Винаги съм се впечатлявала от изобретателността и схемите, които могат да се реализират и винаги съм се учудвала, че това носи някому удовлетворение. Когато говоря с такива хора, които ми твърдят, че правят бизнес, бързо разяснявам, че бизнесът е бял, чист и реален. Останалото е престъпление. Ако се проследи историята на болница "Хигия", ще се установи, че всяка инвестиция в нея е чиста, с доказан произход. Финансовите потоци са ясно дефинирани и видими. Нито баща ми, нито аз сме си позволявали да превърнем служителите ни в престъпници, давайки им заплати в пликчета и превръщайки ги в съучастници в сивата зона. Да правиш бял и чист бизнес в България, да не си корумпиран и да не се опитваш да корумпираш е въпрос на достойнство, с всички рискове, които произтичат от това.

Пациентите на "Хигия" знаят, че персоналът в болницата е усмихнат и отзивчив. Как като мениджър успявате да постигнете това високо ниво на удовлетворение сред служителите?

Никога не съм третирала колегите ми като служители. Те са лекари и аз съм лекар. Ние сме партньори във функционирането на една много добра болница. Искам да отбележа, че медицинските сестри в болница "Хигия" носят аристократизма от миналото на тази професия и стоейки зад гърба на лекарите всъщност осъзнават, че те са лицето на екипите пред пациентите.

Как избирате кой да се присъедини към вашия голям екип? Какви качества търсите и изисквате от своите колеги?

Разпознавам човека още в първите пет минути на разговора ни. В него няма умора от живота и отегчение от професията. Искам човекът да е работен, всеотдаен, да ми предлага новости, да изисква от мен опции, за да се развива. На първо място – искам да е добър човек. И едва след това има значение професионалният му път. Има сестри, които са работили на десет  места, но носят отпечатъка на отегчението и лошите порядки от съответните лечебни структури. Дори да имам нужда от тях – не ги вземам. И обратно, има кадри, които идват и казват, че са готови на всичко, за да имат контакт с пациента и за да усвоят професията в най-добрата ѝ светлина – грижа, отношение, динамика, йерархия. За такива хора винаги съм отворена.

Вие сте едновременно лекар и мениджър. Коя от тези две роли ви доставя по-голямо удоволствие? Срещате ли предизвикателства при съчетаването им?

Не познавам лекар, за който този въпрос да има отговор освен, че няма нищо по-хубаво от това да си лекар. В лека конкуренция може да влезе само ролята ми на майка, но синът ми се очертава твърде самостоятелен и независим, така че да бъда лекар е луксът в живота ми. Никога не съм отричала, че "царската корона" на мениджър ми тежи. Да го кажем така – когато си мениджър, неизбежно някога трябва да вземеш и наказателни решения. Докато когато си лекар се налага и се изисква да вършиш само добро.

Кое е по-трудно – да вземеш решение в операционната зала или решение, от което зависи бъдещето на цяла болница?

Хирургията е върхът на пирамидата в медицината. И го твърдя не защото аз съм хирург, а защото реалността е такава. Сигурна съм, че колегите ми от другите специалности напълно споделят тази теза. В операционната зала нерядко решенията се взимат в рамките на 30 секунди. И зад тях седи отговорност за цял един живот. Това няма нищо общо с мениджърската отговорност за фискална стабилност, инвестиции, заплати, пазарни стратегии. Няма по-съкровени решения от тези в операционната зала. В голям процент от случаите те са окончателни и за тях се искат освен знания и умения, и много силна психика.

Как виждате ролята на изкуствения интелект в съвременната медицина? Как вие, в "Хигия", се подготвяте за въвеждането му в лечебния процес?

Изкуственият интелект се оказва нещо много полезно. Интересни публикации чета. За базиране на AI устройства, които се носят от пациента и анализират биологичните му показатели, като могат да предвидят настъпването на остър инцидент или рецидив в хронично заболяване – като инфаркт, инсулт, пристъп на мигрена, мениеров пристъп и т.н. AI навлиза широко в образната диагностика. Ние имаме ЯМР, който благодарение на AI изработва образите три пъти по-бързо и с около десет  пъти по-висока точност от апаратите без AI. Изкуственият интелект ще навлезе широко в медицината като основа, върху която човешкият фактор ще взема по-бързи и по-точни решения, без да може обаче да бъде заменен в окончателното въздействие върху пациента.

Може ли една частна болница едновременно да бъде успешен бизнес и да поставя пациента пред финансовия резултат?

Очевидно може. "Хигия" съществува 35 години по този начин.

Когато поехте ръководството на "Хигия", бяхте най-младият собственик и мениджър на частни лечебни заведения в България. Как се чувствате в тази роля?

Вече се усещам малко изморена. Може би защото да си собственик означава, че си на работа 24 часа, 365 дни. Искрено се забавлявам, когато ме попитат "Кога излизаш в отпуска?". Този въпрос винаги ме кара да се усмихвам. И да, за зло или за добро, съдбата ми е предизвикателство.
