The founder of Medical Dent Dental Clinic has launched a national campaign to make modern dental care in Bulgaria more accessible

Dr Branimir Kirilov, a leading implantologist, oral surgeon and founder of Medical Dent Dental Clinic, keeps a demanding schedule: treating both straightforward and complex cases, attending conferences and training programmes, and managing a team of professionals. Drawing on this experience, he has identified a growing problem in modern Bulgaria – and he is ready to offer a practical solution.

An increasing number of Bulgarians choose low-cost dental procedures abroad without fully understanding the risks involved. Complications are often unavoidable, and treating them can be both expensive and time-consuming. In response, Dr Kirilov has launched the national campaign "Choose Treatment in Bulgaria", which aims to encourage patients to seek care in Bulgaria, provide more affordable access to highly specialised dental treatment, and help build the next generation of qualified professionals.

What are the main risks facing people who seek lower-cost, highly specialised dental treatment abroad?

When we talk about health tourism, we need to make an important distinction, and that is the starting point. For someone from Scandinavia, for example, where an implant and crown costs around 3,000 euros, the prices charged by a good clinic in Eastern Europe, perhaps half that amount, are extremely attractive. The cost is lower, yet the quality remains high. However, when treatment costs half or even less than it would at a reputable clinic in Bulgaria, the risk of poor-quality care increases significantly. Prices cannot keep falling by half indefinitely.

The second point to bear in mind is that health tourists often receive overtreatment. A patient with periodontal disease, for example, may see an advertisement for a foreign clinic promising that, after a one-week stay in a luxury hotel, they will return home with a perfect white smile. Yet the only way to achieve such a result within a week is to have all their teeth extracted, implants placed and a bridge fitted – rather like amputating the leg of a patient who needs a bypass to improve circulation, and fitting a robotic prosthesis instead.

Periodontal disease requires treatment – in medical terminology, this is known as secondary prevention. However, when we remove a patient's teeth and leave them edentulous, we move them directly into tertiary prevention: the treatment of complications arising from the disease. This comes at considerable cost. The patient pays once for treatment abroad, and a second time when, having effectively become disabled by the loss of their natural teeth, they develop complications related to the implants supporting the bridge. Further treatment follows, often involving the replacement of implants and the fabrication of new bridges. In other words, they enter a cycle of expensive treatment that may continue for years.

Another example: advertisements for low-cost dental treatment may promise a patient with misaligned teeth a completely new smile within a week. Yet orthodontic treatment is a slow process, requiring one to two years and regular visits to the dentist every couple of months. So what actually happens during this one-week "treatment"? The patient's teeth are ground down and covered with white bridges. Five or ten years later, the teeth beneath those bridges may have decayed beyond repair and need to be extracted and replaced with implants. Once again, we are dealing with tertiary prevention.

Primary prevention, which helps prevent disease from developing in the first place, is the least expensive option. Once disease occurs, it must be treated, and this is already more costly. But treating the complications of disease – tertiary prevention – is by far the most expensive stage.

Today, an entire group of Bulgarian patients travelling abroad for cheap dental treatment are effectively placing themselves directly into the category requiring tertiary prevention. In the long term, this could mean vast financial resources spent on treating complications, alongside serious deterioration in their overall oral health.

Is this what motivated you to launch the "Choose Treatment in Bulgaria" campaign?

Yes. Every day and every week I encounter patients suffering the consequences of health tourism. They now face follow-up treatment involving multiple interventions and corrective procedures that can take 12 to 18 months to complete. These are highly demanding in terms of time, resources and expertise, and far more complex to carry out than the original treatment should have been.

It is important to understand that, at some point in the future, these complications will arrive like a wave. For the first three, five or even ten years, patients who have undergone treatment abroad may continue to live in relative comfort. But after that, complications will begin to emerge, and we will see a growing number of cases requiring extensive corrective work. Treating them will require enough highly qualified specialists, because these are not cases that every dentist is capable of managing.

What is the core philosophy behind the "Choose Treatment in Bulgaria" campaign?

Our goal is to raise awareness of the problem. People need to understand that there are established treatment protocols, and that these must be followed if treatment is to be genuinely effective.

A possible solution at national level is to create opportunities for young dentists to undertake specialist training in quality Bulgarian dental clinics and, during that period, provide treatment at reduced prices.

Medical Dent, for example, is licensed as a postgraduate training centre for dentists in several specialities. The programme's theoretical component and examinations are conducted through Sofia University. Such training opportunities allow dentists in specialist training to treat patients at prices lower than standard commercial rates, which reflect a clinician's qualifications and the years of training they have invested in their career.

A similar approach could be developed into a national programme under which dental clinics accredited as specialist training centres provide treatment below market rates while maintaining high standards. This would give patients seeking affordable care an informed choice and a viable alternative, while allowing trainee specialists to develop their skills and expertise.

However, such a solution would need to be implemented at national level, which is why we are seeking dialogue with the Ministry of Health, the Ministry of Education, the Bulgarian Dental Association and other relevant institutions and organisations. We want to engage with them on these issues and work together to find solutions.

Some people might argue that we live in a market economy and that it is only natural for patients to seek lower prices. That is true. The problem is that most patients, not being medical professionals, do not have enough information to make informed decisions. As a result, they are often drawn in by appealing offers and only discover the consequences years later.

How are you working to close this information gap?

We regularly publish information on our dedicated website, www.lekuvaisevbulgaria.bg, and through our Facebook and Instagram accounts. We are expanding our network of partner dentists and clinics who share this information with their patients, and establishing contacts with media outlets. In short, we are working across every available communication channel.

Работа върху стандартни и предизвикателни случаи, конференции, обучения, организация на екип от професионалисти: работното всекидневие на д-р Бранимир Кирилов, водещ имплантолог, орален хирург и създател на дентална клиника Medical Dent, е повече от натоварено. Но тъкмо от висотата на професионалните си постижения, д-р Кирилов не само е в състояние да забележи един проблем, който набира все по-голяма скорост в съвременна България. Той е готов да предложи адекватно решение.

Какъв е проблемът? Все повече българи избират евтини дентални процедури в чужбина, но не са наясно с рисковете. Усложненията са неизбежни, а лечението им е скъпо и времеемко. В търсене на решение, д-р Кирилов стартира националната кампания "Лекувай се в България". Целта ѝ е да задържи пациентите в България, да предложи по-достъпна високоспециализирана дентална грижа и да помогне за изграждането на следващото поколение квалифицирани професионалисти.

Какви са основните рискове пред хората, които търсят по-евтино високоспециализирано дентално лечение в чужбина?

Когато говорим за здравен туризъм, трябва да направим една разлика – и тя е в изходното ниво. За човек от Скандинавия, например, където лечение с имплант и коронка струва около 3000 евро, цените в добра клиника в Източна Европа, които са наполовина по-ниски, са изключително привлекателни. Цената е по-добра, а качеството е високо. Но когато търсиш цена, която е наполовина или дори по-ниска от тази в добра клиника в България – тогава рискът да получиш лошо лечение значително се повишава. Цената не може постоянно да пада двойно.

Второто, което трябва да се има предвид е, че при здравния туризъм пациентите обикновено получават свръхлечение. Например, когато пациент с пародонтално заболяване види реклама за клиника в чужбина, която обещава, че след едноседмичен престой с луксозен хотел, с всичките му удобства, той ще се върне у дома с бяла усмивка. Но единственият начин един пациент с пародонтално заболяване да получи бяла усмивка за една седмица е да му се извадят всички зъби, да му се сложат импланти и да му се направи мост върху имплантите. Все едно на пациент, който има нужда от байпас на крака, за да се кръвоснабдява по-добре, да му се отреже кракът и да му се сложи роботизирана протеза.

Пародонталното заболяване изисква лечение – по медицинската терминология това е вторична профилактика. Но когато извадим зъбите на пациента и го обеззъбим, ние го пращаме директно в третична профилактика - лечение на усложненията от заболяването. Това е огромна цена, която пациентът плаща веднъж при лечението си в чужбина, и втори път, когато вече е инвалидизиран и получи усложнение от имплантите на моста. И той отново трябва да премине лечение, което отново е свързано със смяна на импланти и нови мостове. Тоест, той оттук натам влиза в един цикъл на много скъпо лечение.

Рекламата на евтино дентално лечение може да предложи изцяло нова усмивка за седмица на пациент с криви зъби. Но ортодонтското лечение е бавен процес, изисква година-две и редовни посещения при зъболекаря на всеки два месеца. Какво се получава по време на "лечението" за една седмица? Зъбите на пациента се изпиляват и върху тях се слагат бели мостове. След пет-десет години, зъбите под мостовете ще са изгнили. Ще трябда да се извадят и да е поставят импланти. Отново отиваме в третична профилактика.

Най-евтина е първичната профилактика, която предпазва от заболяване. Когато възникне заболяване, то трябва да се лекува – това вече е по-скъпо от първичната профилактика. Но когато лекуваме усложненията от заболяването – тоест правим третична профилактика – е най-скъпо.

А сега цяла една група пациенти от България, които ходят на здравен туризъм, директно се нуждае от третична профилактика. Потенциално в бъдеще това означава огромен финансов ресурс за лечение на усложнения на заболяванията и сериозно увреждане на здравословното им състояние.

Това ли ви мотивира да създадете кампанията "Лекувай се в България"?

Да. Всеки ден и всяка седмица се сблъсквам с пациенти, които са пострадали от здравен туризъм. И сега са изправени пред последващо лечение с многократни интервенции и преработки, които отнемат година до година и половина и са много времеемки, ресурсоемки и много по-тежки за изпълнение.

Имайте предвид, че в един момент тези усложнения ще дойдат като вълна. Защото за три, пет, десет години пациентите на здравния туризъм ще живеят в относителен комфорт. Но след това ще започнат усложненията. Ще дойде вълна от случаи, които се нуждаят от преработка. И лечението ще изисква достатъчнен брой квалифицирани специалисти. Защото не всеки зъболекар може да се справи с тях.

Каква е основната философия на "Лекувай се в България"?

Целта ни е да дадем гласност на проблема. Хората трябва да бъдат информирани, че има лечебни протоколи и те трябва да бъдат спазени, за да имаме реално лечение.

Едно решение на национално ниво е да се предостави възможност млади зъболекари да специализират в лечебни заведения в България и по време на специализацията си да предоставят лечение на по-ниски цени.

Медикъл Дент, например, е лицензирана като център за следдипломна квалификация за зъболекари по няколко специалности. Теоретичната част и изпитът им са в Софийския университет. Наличието на тази възможност ще позволи на специализиращи зъболекари в съответните клиники да лекуват паициенти на по-ниски цени от чисто търговската цена. Търговската цена се образува върху квалификацията на лекаря и курсовете, които е посещавал и за които е плащал.

Подобна политика може да стане национална програма, в която зъболекарски клиники, сключили договори да бъдат център за специализация, лекуват пациентите на цени, които са по-ниски от пазарните, но предоставят качество. Така хем, пациентите, които търсят по-ниската цена, ще имат информиран избор и алтернатива, хем специализантите ще имат възможност да се изградят като професионалисти.

Такова решение обаче трябва да бъде постигнато на национално ниво. Затова търсим контакти с Министерството на здравеопазването, с МОН, Българския зъболекарски съюз и други институции. Искаме да влезем в комуникация с тях по тези въпроси и да търсим отговор заедно.

Някои биха казали: намираме се в пазарна икономика, нормално е хората да търсят по-ниските цени, това е пазарно търсене. Да, но обикновените пациенти нямат достатъчно информация – те не са лекари. И често стават жертва на добри оферти, но разбират проблема след няколко години.

Направихме специална уеб страница www.lekuvaisevbulgaria.bg, публикуваме редовно информация в нея и в нашите Facebook и Instagram акаунти. Разширяваме мрежата си от партниращи зъболекари и клиники, които да споделят информацията. Търсим контакти с медии. Работим по всички комуникационни канали.
